Overview
Cleveland Clinic Children’s School–Based Health program offers a mobile, full-service pediatric office staffed with our healthcare professionals. For the school districts we've partnered with, the mobile unit will visit them regularly throughout the school year to provide any needed care. Should your child require healthcare when not at school, our School–Based Health staff or your child’s Cleveland Clinic primary care practitioner can see your child virtually during regular business hours. This service is available through MyChart, which can be accessed online or through your mobile device.
The mobile unit provides voluntary, comprehensive healthcare services to students from kindergarten through 12th grade. We know that children who are healthy are best equipped to learn and excel in the classroom. Cleveland Clinic Children's is committed to ensuring that all students receive high-quality, comprehensive healthcare with the added convenience of receiving care on the mobile unit, virtually in school or at home. The School–Based Health Center will collaborate with your child's existing healthcare provider or can act as a primary care provider for them. If your child needs care outside of our operating hours, you can make an appointment at any Cleveland Clinic Children’s location.
Pediatric Mental Health Services
Are you worried about your child’s mental health? Find support with the School-Based Health Care program. Pediatric psychologist Gerard Banez, PhD, provides mental health treatment and assessment. Children can be seen in-person or virtually (from school or home).
Enrollment
Parents or legal guardians must complete enrollment in order for their child to be seen by the School-Based Health Care Team. If your child does not have a completed enrollment on file, they will be unable to use the program.
Enrollment is voluntary and can be discontinued at any time. There are no eligibility requirements except that your child must currently attend a school district where a School-Based Health Center is located. To arrange for your child to be seen at the School-Based Health Center, you may call your child’s school nurse, our mobile health center staff or send a note. Parents are welcome and encouraged to accompany their children. If your child is too ill to attend school and you would like them to be seen by our staff, you may call the school nurse to arrange a same-day appointment.

FAQs
Who will my child see in the mobile health center?
A Cleveland Clinic Children’s Pediatrician or Pediatric Nurse Practitioner (NP) will be on site to diagnose and treat illnesses and prescribe medications. There will also be a Licensed Practical Nurse (LPN) present as well as a Medical Assistant (MA) at some sites.
Who would have access to my child’s medical records?
All medical records are confidential, as they are part of the Cleveland Clinic medical records system. Before we can share any records, parents/legal guardian must sign the Release Form.
Are school nurse services still available?
Yes, the school nurse will continue to provide your child with routine services, regardless of his or her enrollment in the School-Based Health Center.

The Medical Clinic at Lakewood High School was established to serve the needs of the students and employees of Lakewood City Schools. Our goal is to ensure everyone stays healthy and has access to the right care when they need it most.

A School-Based Health Enrollment Packet must be completed. You can receive your enrollment packet by:
- Contacting your school nurse or health aide
- Downloading the Lakewood City School District enrollment packet
- Stopping by the Medical Clinic at Lakewood High School at 14100 Franklin Boulevard
Once the packet is complete, please return to your school nurse, drop it off at the medical clinic or mail directly to the clinic at 14100 Franklin Boulevard, Lakewood, OH 44107.
Once a student is enrolled, how should they schedule an appointment at the medical clinic?
If a student is ill during the school day, they will see their school nurse first who will then determine if an appointment is needed and facilitate the process.

For any follow up medical needs, we will refer you back to your Primary Care Provider after you see us. If you do not have a primary care provider we would be happy to be your primary care provider.

Payment
If you have insurance, your insurance will be billed for services. Co-pays are also billed to your insurance. If you don’t have insurance, our financial counselors can help you get insurance for your child. Financial assistance is available for those who qualify.

Cleveland Clinic Children's mobile health unit offers world-class services for your child, including:
- Complete physical examinations (may be used for sports physicals, camp, college, work authorizations, etc.)
- Comprehensive healthcare, including diagnosis and treatment of acute and chronic illness and reproductive healthcare

Epinephrine Auto Injector Prescription Program
Cleveland Clinic Children’s School-Based Health Care offers non-individual-specific epinephrine auto-injector prescriptions for schools as well as training for school employees (in accordance with Ohio House Bill 296 effective April 2014).
Epinephrine injection is used along with emergency medical treatment to treat life-threatening allergic reactions caused by insect bites or stings, foods, medications, latex and other causes. If given in time, this drug can reverse the life-threatening symptoms of anaphylaxis.
